Job Overview

This is a part-time position with flexible scheduling based on staff schedules and individual preferences. Staff can expect to work a maximum of 30 hours per week; staff will coordinate their schedule and the total number of hours worked with supervisors. Availability on evenings and weekends is strongly preferred. This job requires staff to use his or her own car to travel to various locations within Lackawanna/Susquehanna County. The job of a Community Support Professional is to support individuals with Intellectual or Developmental disabilities in the community. Staff will provide transportation and act as a companion/support for individuals. This may involve assisting with Activities of Daily Living; accompanying an individual to preferred activities like bowling; assisting an individual with making healthy choices while grocery shopping; etc.

Duties and Responsibilities include but are not limited to:

  • • Provides in-home or community based supports to an individual and/or family specific to stated needs of such persons.
    • Be flexible, within given parameters, to establish a work schedule based on individual and family needs and authorized hours. Do not exceed authorized hours as per individual’s ISP.
    • When appropriate, will provide transportation to and accompany the individual on community-based activities.
    • Reports to the Director of Community Support Services any change of work schedule, individual/family discontinuation of services or any temporary alteration of schedule or authorized hours.
    • Submit Progress notes that document activities that reflect consumer outcomes as required.
    • Implements specific ISP outcomes as outlined by the individual’s program team including progress notes.
    • Maintains documentation as required by the Director of Community Support Services.
    • Submits timesheets/waiver service progress notes and session notes as prescribed by the finance department of St. Joseph’s Center and the Director Community Support Services.
    • Provides all insurance, vehicle. and drivers information as required by Provider Monitoring.
    • Performs other related tasks as assigned and remains flexible to the diverse needs of the facility.


  • Be knowledgeable or capable of learning the philosophy of supporting individuals and/or families in their home environment.
  • Be knowledgeable or capable of learning that support services are afforded in a person’s home, and that interaction with the individual/family members will be in a manner in which ensures respect, dignity, privacy and human rights for all involved.
  • Must be capable of working independently with an individual/family as well as cooperatively with those persons and other agencies.


  • High School Diploma or GED
  • Valid Pennsylvania Driver’s License
  • Access to a privately owned vehicle
  • Experience preferred
